Room 319-Observed live roaches in the microwave area. Observed dead bugs in the restroom lighting fixture and behind the bed. Room 326- Observed dead bugs in the window seal Room 107- Observed dead roaches below the refrigerator Room 123- Dead bugs behind the refrigerator
511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program.

Room 306- Observed hair on pillow covers. Stains on the top cover. Tears on linen. Room 123- Pillow covers contained yellow stains Room 107- Stain on the side of the mattress cover. Pillow and pillow cover contained yellow stains.
Room-326- Observed several pieces of large hair on the mattress cover, linens, pillows and interior of the microwave
Room 319- Observed yellow staining on pillow and brown stains on the side of the mattress cover

Room 319- Observed dirt build up on the shower floor. Hair also observed on the floor Room 306- Hair observed in the restroom light fixture. Room 326- Hair observed on towels. Rust observed on the air vents. Room 306- Hair observed on the floor of the shower. Room 123- Stains observed on shower curtain

Room 107- Observed ground coffee on the floor beneath the refrigerator. Also observed several dog hairs on the floor beneath the head of the bed
15 - ice from approved source; properly handled, supplied and stored; equipment approved/clean
Regulation: 511-6-2-.07(8) - ice supply
Observed the ice machine on the first floor with an excessive amount of black mold-like build up. The ice machine chute must be cleaned weekly.
511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice
