Room 108-Observed build up and toys beneath chair pillows. Stain observed on table Room 119_Red stain observed on the inside of the reach in cooler. Room 210- Desk lamp observed in bad repair. 511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Room 108- Observed black specs on linen Room 119- Large oil stain observed on comforter. Hair and debris observed on mattress cover. Large/heavy red and yellow stains on mattress cover. PIC advised to take the room offline until linen is replaced with new linen. Room 210- Observed burn holes on comforter, stains on pillow, holes in the mattress, heavy stains on mattress cover. PIC advised to not rent the room until all linen is replaced with new linen.
511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists

Observed an odor in rooms 108 and 210. PIC will mop the rooms and open the door to ventilate the room

Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

Observed an empty beer can below the bed in room 210. Also observed a dirty towel behind the door on the floor in the restroom. The ceiling was also observed in bad repair.

Observed signs of mold on the interion housing of the ice machine. PIC advised to deep clean the ice machine by 6/4/2026
511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice
32 - separate, structurally sound, clean, and in good repair: dryers vented and adequate lighting
Regulation: 511-6-2-.17(1) - laundry facilities
Observed 3 big holes on the walls in the laundry room and the vent system not sealed properly on the ceiling. There is also a thick accumulation of dust on the walls behind the washer and dryer. Employee food stored next to clean linen. Chemical bottle observed with no label. The holes must be sealed and painted to make the walls smooth, easily cleanable, and nonabsorbent. The walls must be cleaned and repaired
