Observed the gas fired equipment form was expired. PIC will have the gas fired equipment inspected by 1/22/2026 due to the gas odor that was observed in the laundry room.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Room 404- Mattress contained heavy stains. Room 419- Heavy stain on the mattress Room 300- Pillow cover stained. Mattress and mattress cover stained Room 202- Observed stains on pillow case. Heavy stains and mildew on mattresses. Observed heavy gas odor in the laundry room. PIC advised to contact a licensed plumber to have the gas odor checked and gas fired equipment form updated.

511-6-2-.14(3) - Room Ventilation Ventilation shall be provided for all rooms. Where ventilation is provided by means of windows, they shall open directly to the outside air and the openable window area of each room shall be not less than 1/20 of the floor area served. Where ventilation is provided by other means, it shall comply with the requirements of the current International Mechanical Code with Georgia Amendments or future subsequent revisions adopted thereafter to provide comfortable living conditions, remove objectionable odor and fumes, and prevent excessive condensation. Ventilation systems shall comply with applicable State and local fire prevention requirements and building codes

Second floor, Fourth Floor-Observed black build up on the interior of the ice machine

Room 104: Observed the draperies off track and the side of the bed with a green stain and dust build up.

Room 212: Observed stains on the ironing board cover.

Room 304: Observed stains inside of the microwave, debris on the interior drawer of the suitcase shelf, and stains on the ironing board cover.

Room 415: Observed the draperies with small spots and debris on the interior surface of the drawers.

Room 215: Observed the ironing board cover stained.

Room 305: Observed the lamp covers with a thick mildew stain. Also, the draperies and the ironing board cover had stains

Room 212: Observed the pillowcase with stains.

Room 215: Observed stains on the pillowcase, mattress, and pillow. Observed the pillow in bad repair.

Room 305: Observed pillowcase with stains, stains on the mattress cover, and a big stain on both mattresses.

Room 304: Observed a pillow in bad repair, pillowcase with stains, black mold like stains on another pillow, and stains on mattress and mattress cover.

Room 415: Observed stains on pillowcase and pillow and big stains on both mattresses

Observed debris and clutter on the 2nd and 3rd floors in the chute room and the the storage rooms.

Also, observed the floor tile in bad repair in the room that housing the ice machine. The tile must be repaired and properly grouted to the floor

Observed the current inspection report not posted. An old inspection report was posted from 2024. PIC stated that he thought that was the most recent report.

511-6-2-.05(7) - Inspection Posting The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed the current permit expired. The facility has a balance of $225.00 that is due to the Henry County Environmental Office by 8/6/25. A legal notice was reviewed and signed by the PIC.

511-6-2-.04(1)a - Permit Display Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. (b) To qualify for a permit, an applicant shall:

1. Be an owner of the tourist accommodation;
2. Allow access to the tourist accommodation;
3. Provide all information as required on the application and pay all applicable local and state fees referenced in DPH Rule 511-6-2-.22 at the time the application is submitted and;
4. Score 100% on the permitting inspection for newly constructed facilities

All rooms bathroom's floor are in need of a deep cleaning underneath the sinks (debris, dirt, and hair build up).

Room 104: The bathroom door is off track and is very hard to open and close. Observed big pieces of hair on the shower's floor. Also, the shower floor is cracking and has to be repaired. Observed a stain on a towel.

Room 212: Observed hair in the tub and yellow stains on the ceiling above the tub. The leak must be fixed and the stain properly sealed.

Room 305: Observed the toilet with a cracked lid, tub with soap residue, hair on a towel, and a stain on the sink's counter.

Room 304: Observed the tub's fixtures loose, stained towels and the caulking above the shower with a black mold-like buildup.

Room 415: Observed the toilet not flushing (chain broke) and the sink's counter wet

Observed the AC unit not working on the 2nd, 3rd, and 4th floors (common area) also in the back office area of the hotel. Manager stated that a contracting company has been working on the unit, but continues to receive an error message. The work is ongoing. Please email all copies of the service reports until the issue has been fixed. The company is Cool MCA HVAC LLC was at the facility on today (7/31).

• Motor replaced on the 3rd floor and other issues and being fixed ongoing. The problem has been going on for about 3 weeks.

Also observed dust build up on air conditioning filters in rooms 104 and 305

Observed no current annual gas-fired equipment inspection on file. Please have gas-fired equipment inspected by 1/13/2025 and email a copy to keundra.golden@dph.ga.gov

Room 421: Hair in bedding material. Room 402: Stained mattress and hair on mattress cover. Dog hair and stains on sofa bed cushions. Room 314: Stains and hair in bedding material. Room 421: Observed possible water damage above the shower. Room 412: Stained walls and tear/paint chip above the shower. Room 310: Bathroom ceiling in need of repair due to small cracks. Debris found under the beds. Laundry chute room on the 4th floor contained debris on the floor. Women's restroom and ice machine room on the first floor was observed to have water damage on the ceilings and walls causing the wall paper to separate from the wall

All rooms inspected were observed to have dust build up on air conditioning filters

Observed ice machines on 1st and 3rd floor with mold like substances on the interior surface of the ice chute. Observed ice machine with water and mold like substance build up on the floor. Please discontinue use of the 1st floor ice machine until repairs have been made and the water and mold has been cleaned.

Observed in Room 122 stained top sheet, pillowcase, pillows, mattress. Also had hair on the pillowcase. Room 207: Stained pillowcases, pillows, mattress protector and both mattresses. Also had hair on mattress. Room 205: hole on sheet, stained pillows, pillowcases, and both mattresses. Also had hair on mattress cover and pillowcase. Room 301: Stained pillow cover, pillow, mattress cover, comforter and mattress. Room 319: Stained fitted sheet, pillows. and pillowcase. Also had hair on pillow protector and mattress protector. room 418: Stained comforter. pillow, pillowcase, and mattress. Room 420: Stained top sheet, mattress, pillows and pillowcase and mattress protector

Observed the shower tile cracked , sink counter damaged, shower bar damaged, and hair on towels and floor in room 122.Room 207: Observed hair on toilet and floors and the towels and hair dryer was stained. Room 205: Observed hair on floor and the hair dryer stained. Room 301:Observed hair on floor and stained towels. Room 319: Observed hair on floor, hair dryer, and towels. Room 418: Towels stained and hair on towels and floor. Room 420: Toilet stained

Observed in Room 207 the shower head with mold and the vent above toilet is dusty. In Room 205 mold on shower head and shower curtain, shower bar damaged and vent dusty. In Room 319 mold on shower head and shower needs to be re caulked. Room 418 mold on shower head and room 420 shower needs to be re caulked

Observed in Room 122 floors with hair. Room 207 food debris and hair on floor. Room 205 hair on floor and baseboards need to be fixed. Room 301 found medicine on the floor. Room 319 fabric on floor and baseboard damaged in closet. Room 420 found a toy on the floor behind the chair. (Repeat)
