Observed the CFSM no longer works at the establishment. GM (Ariel Davis) advised that she is in the process of obtaining her certification. PIC will email a copy of the certification to robert.lewis1@dph.ga.gov by 2/13/2026.
511-6-1.03(3)(a) - Food Safety Manager Certification (Pf) Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) as specified in paragraph (b) of this subsection to ensure food safety is being managed within the food service establishment during all hours of operation as specified within paragraph (d) of this subsection. 12B - personal cleanliness

Team members not wearing proper hair restraints while preparing food on the front line.
511-6-1.03(5)(i) - Hair Restraints (C) (i) Hair Restraints.

Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.

This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.
15B - warewashing facilities: installed, maintained, used; test strips

Observed the location does not have quaternary ammonium test strips to test sanitizer in the three compartment sink. CA- PIC will obtain quaternary ammonium test strips by 2/4/26.

17B - garbage/refuse properly disposed; facilities maintained

Observed the garbage dumpster is missing both lids. Also observed trash around the dumpster. The dumpster must be replaced by 2/5/2026. A copy of the completed service work order must be emailed to Robert.Lewis1@dph.ga.gov by 2/4/2026.
511-6-1.06(5)(n) - Covering Receptacles (C) (n) Covering Receptacles. Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered:
- Inside the food service establishment if the receptacles and units:
(i) Contain food residue and are not in continuous use; or
(ii) After they are filled; and
- With tight-fitting lids or doors if kept outside the food service establishment.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed the ceiling tile in bad repair above the hot water heater and walk in freezer. Ceiling tile must be properly installed to create a washable, non absorbent surface in the mop sink/storage area and to prevent pest activity. The PIC did not know why the ceiling tile was not properly installed. CA-PIC will have the ceiling tile installed by 1/20/25.

(a) Good Repair. All physical facilities shall be maintained in good repair.
(b) Cleaning, Frequency and Restrictions.

The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing.
