Observed food items stored in reach-in freezer exposed to ice accumulation. 511-6-1.04(4)(q) - Food Storage (C) (q) Food Storage.

1. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food:

(i) In a clean, dry location;

(ii) Where it is not exposed to splash, dust, or other contamination; and

(iii) At least 6 inches (15 cm) above the floor.

2. Food in packages and working containers may be stored less than 6 inches (15 cm) above the floor on case lot handling equipment if the equipment can be moved by hand or by conveniently available apparatuses such as hand trucks and forklifts.

3. Pressurized beverage containers, cased food in waterproof containers such as bottles or cans, and milk containers in plastic crates may be stored on a floor that is clean and not exposed to floor moisture.

Observed reach-in freezer with excessive accumulation of ice. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.

3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed outside dumpster lid in disrepair and drain plug missing. 511-6-1.06(5)(f) - Outside Receptacles, design & construction (C) (f) Outside Receptacles.

1. Receptacles and waste handling units for refuse, recyclables, and returnables used with materials containing food residue and used outside the food service establishment shall be designed and constructed to have tight-fitting lids, doors, or covers.

2. Receptacles and waste handling units for refuse and recyclables such as an on-site compactor shall be installed so that accumulation of debris and insect and rodent attraction and harborage are minimized and effective cleaning is facilitated around and, if the unit is not installed flush with the base pad, under the unit.

Observed floors with standing water accumulation throughout the facility.

Observed ice bins left uncovered while not in use.

Observed multiple jugs of milk left uncovered/open on the countertop and inside the reach in cooler. PIC covered all exposed items. (COS) (Repeat)

Observed wiping cloths not in use laying on the counter tops. PIC removed wet towels and placed in the sanitizer bins. (COS)

Observed several gnats flying throughout the facility. Pest control will need to treat by June 6, 2023. Please email a copy of report.

Observed the permit posted with a date of 1/1/2019.

Observed the reach in cooler with condensation and old milk spilled on the bottom. This clean in place equipment will need to be cleaned properly.

Observed both restrooms (unisex) with garbage bins with no cover.

Observed the floors in the front-end area in need of a good cleaning.
