6-1B - proper hot holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; hot holding (p)
Observed containers of vegetable soup stored in the hot holding steam well did not maintain a temperature of 135 degrees or higher. The vegetable soup was observed at 126 degrees f. PIC corrected onsite by reheating to 165 degrees f.
511-6-1.04(6)(f) - Time/Temperature Control for Safety; Hot Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. Observed the mop sink faucet leaking causing water accumulation on the floor around the mop sink. PIC repair the plumbing leak by 2/10/26.

13A - posted: permit/inspection/choking poster/handwashing

Observed employee must wash hands before returning to work signs not provided in the men's and ladies restroom. 511-6-1.07(3)(d) - Handwashing Signage (C) (d) Handwashing Signage. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees.
14B - utensils, equipment and linens: properly stored, dried, handled

Observed a grill press stored on an unclean surface below the fry hot holding station. PIC advised to store in a clean/dry location.

(iii) At least 6 inches (15 centimeters) above the floor. 2. Clean equipment and utensils shall be stored as specified under paragraph 1 of this subsection and shall be stored:
(i) In a self-draining position that allows air drying; and
(ii) Covered or inverted. 4. Items that are kept in closed packages may be stored less than 6 inches (15 centimeters) above the floor on dollies, pallets, racks, and skids that are designed as specified under subsection (2)(hh) of this Rule.

Observed single use articles (lids) stored on the floor of the dry storage room.

15C - nonfood-contact surfaces clean

Observed build up (saw dust) and black accumulation in the cabinets below the drink stations. There were cookies also stored in the cabinet.

The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.
- Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17C - physical facilities installed, maintained, and clean

Observed the floor in the dry storage room in bad repair. There were several area that contained floor tile in disrepair. PIC will have the floors repaired by 2/10/26 to prevent injury and water accumulation. There was also rust build up in the CO2 tank storage area.
17D - adequate ventilation and lighting; designated areas used

Observed build up on vents in the dry storage and clean dish storage area. PIC will have the vents clean by 2/03/26 to prevent potential contamination.
