Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed employees without proper hair or beard restraints. Manager passed out hats for employees to cover their hair and was informed that the male employee with a beard must cut it to half and inch or purchase a beard net.

Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.

This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.

Observed flies in the back of the kitchen.

Observed walk in cooler door unable to self-close.

Observed mold like substance and dirt build up around the caulking of the 3 compartment sink. Please have old caulk removed, clean the area, and re-caulk the 3 compartment sink. Observed storing shelves throughout the kitchen with heavy dirt and food debris. Observed employee microwave and reach in cooler in need of cleaning.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed ceilings in need of cleaning due to grease build up. Observed walls in need of cleaning due to dirt build up around the baseboards and underneath the 3 compartment sink. Observed holes in the wall of the employee restroom. Observed floor tiles in disrepair near the ware washing area.
