Room 416: Observed long hairs on linens. Room 308: Observed hair on mattress cover. Room 209: Observed hair on mattress cover. Room 218: Observed long hairs on linens. Room 121: Observed hair on linens. Room 124: Observed hair on linens. Room 125: Observed several long strands of hair on mattress covers of both beds

Room 416: Observed hair on towels. Room 413: Observed door stop not properly placed onto door and hair on towel. Room 320: Observed squeeze bottle shampoo residue in tub. Room 218: Observed hair on toilet and hair on towels. Room 121: Observed grey staining on shower floor. Room 124: Observed cracked floor tile and hair on towel. Room 125: Observed dust accumulation on vent and hair on towels

Room 308: Observed pills on floor behind bed. Room 121: Observed dust accumulation and false nails on floor behind beds. Room 124: Observed phone charger on floor behind bed

Observed accumulation in interior of ice chute

Two chemical bottles stored with wrapped paper cups and no spray triggers on two bottles on housekeeping cart on 4th floor. On 3rd floor employee chemicals were observed stored next to wrapped paper cups on housekeeping cart and box of pillows on floor in house keeping storage room. On 2nd floor multiple bottles of chemicals were observed unlabeled on cart and in housekeeping storage room, and lids not chemicals in storage room

Observed hair and stained linen in all rooms inspected. Pet hair was found in multiple rooms inspected. Due to the hotel being pet friendly, all linen and mattress covers must be removed and cleaned after each rental. Observed hair and debris behind the bed in room 216.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed the inspection report and permit not posted 511-6-2-.04(1)a - Permit Display Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. (b) To qualify for a permit, an applicant shall:

1. Be an owner of the tourist accommodation;
2. Allow access to the tourist accommodation;
3. Provide all information as required on the application and pay all applicable local and state fees referenced in DPH Rule 511-6-2-.22 at the time the application is submitted and;
4. Score 100% on the permitting inspection for newly constructed facilities.

511-6-2-.05(7) - Inspection Posting The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed chemical spray bottles in the housekeeping card and the laundry room without a common name label. CA- PIC will ensure that all chemical bottles are properly labeled
