Manager could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food.
12A - contamination prevented during food preparation, storage, display

Observed boxes of food stored directly on the floor in the dry storage room. Employee corrected on site by placing box of food 6 inches above the floor.

17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed dust accumulating on the ceiling tiles in the kitchen. Observed damaged ceiling tile in unisex restroom above the hand sink. Manager was advised that restroom ceiling tiles should be a smooth and cleanable service like those in the kitchen. Since new ownership was approved with the acoustic ceiling tile in the restroom, manager was advised to replace ceiling tiles with vinyl gypsum tiles as they become damaged over time.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(5)(d) - cleaning ventilation system, nuisance & discharge prohibition, cleaned in way not to cause contamination or create a public health hazard (c)
Observed dust and excessive rust build up on air vents in the main kitchen. Manager was advised to have air vents deeply cleaned and repainted with light colored food grade paint.
