Observed staff members wearing jewelry (watches) during food preparation. PIC corrected on site by having all jewelry removed.

511-6-1.03(5)(g) - Jewelry (C) (g) Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.

Observed dirty dishes and equipment stored above clean dishes. PIC will remove and clean all dirty dishes to prevent contamination. 511-6-1.05(10)(e)1,2,4 - Equipment, Utensil, Linens, stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles.

1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored:

(i) In a clean, dry location;

(ii) Where they are not exposed to splash, dust, or other contamination; and

(iii) At least 6 inches (15 centimeters) above the floor. 2. Clean equipment and utensils shall be stored as specified under paragraph 1 of this subsection and shall be stored:

(i) In a self-draining position that allows air drying; and

(ii) Covered or inverted. 4. Items that are kept in closed packages may be stored less than 6 inches (15 centimeters) above the floor on dollies, pallets, racks, and skids that are designed as specified under subsection (2)(hh) of this Rule.

Observed the restroom does not have a trash can and/or a covered trash can for sanitary napkins. PIC will install a trash can with a lid by 03/20/2026.

511-6-1.06(5)(h) - Toilet Room Receptacle, Covered (C) (h) Toilet Room Receptacle, Covered. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

Observed a white build up inside of the dishmachine. PIC advised the dishmachine has not been used for five years. PIC advised to maintain the cleanliness of the dishmachine.

Observed dead and live bugs in the dishmachine. Dead roaches and live spiders were observed. This is a repeat violation. PIC will contact Ms. Brown and maintenance.

Observed mold build up on gaskets in a reach in cooler that is being removed. PIC advised that the cooler has not been used in five years.

Observed glue trap underneath the dry storage shelf with at least 2 species of dead insects accumulating on the trap (4 cockroaches and several ants). Observed dead roach in the dish washing room. All dead insect must be removed at a frequency to prevent accumulation . Manager removed bugs.

Observed rust on air vents in the main kitchen. Clean and repaint with food grade paint.

Observed American cheese held past discard date (3/18/24). Manager corrected on site by discarding item (COS).

observed American cheese slices in the walk in cooler that was out of date. The date on the cheese was 8/29/2023, the staff did correct on site by discarding the food product. Staff member stated she had not had a chance to pull the food product out.
