Observed an accumulation of food debris on reach in cooler handles and cooler drawers. Additionally observed a build up of food debris inside of cooler that is not currently in use per conversation with PIC. COS: PIC instructed employees to begin wiping down the coolers and stated that they will have the other cooler cleaned out by the closing shift. CA: Nonfood contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris.

Observed PIC was unable to provide a verifiable method to the employee health policy. PIC continued to reiterate that everything is electronic. When asked to access the electronic method PIC was unable to show the signed forms due to her not having access to click on the hyper link that shows signed. PIC stated that when the employees clock in they are asked if they are sick. Was able to see that this showed the 5 symptoms but none of the diagnosis. PIC became frustrated and stated this is bullshit and none of the other stores have ever been asked this. PIC eventually found a old copy of the DPH employee health policy that was stored in a folder with an older inspection. PIC found a copy of an blank older version of "Panera's" employee health policy that was missing 2 of reportable diagnosis (typhoid and shigellos). CA: Establishment will need to provide a verifiable method for all staff members or find out how to access the signed electronic copies.

Observed scoop sitting with its handle inside of the salad located on the prep top. COS: PIC took scoop and placed it on the side of the pan with the scoop sitting out of the food. CA: During pauses in food preparation, if scoops are to be stored inside of the food item the handle must be always be stored above the food item.
