Observed front hand sink cleanser was not available. Per conversation with PIC the dispenser is broke and does not release the hand soap so they use the one in the back of the store. Informed PIC that hand sinks must have cleanser at them at all times. COS: PIC contacted the Store Manger and she returned with hand soap to place at sink until dispenser could be fixed.

Observed facility was not following approved variance for breadstick sauce. Inspector observed printed label for the sauce, had a discard date of 04/02/25. Per the approved variance from the state the sauce shall be discarded by close of business. Inquired if the facility had a copy of the variance procedure in the establishment. PIC unsure and reached out to District Manager. Informed both the PIC and District Manager that a copy of the approved variance must be kept on file to ensure proper training in regards to the approved variance procedures. COS: PIC checked the machine and found a sticker that stated "ambient" for the breadstick sauce. PIC placed corrected sticker on sauce. CA: Copy of variance provided at last inspection. Inspector will provide another copy of variance agreement.

Observed back door was not self closing. When inspector opened the door it would not close on its own unless shut. Observed front door to the facility had a sign that instructed patrons to close it manually. Front door also was not tight fitting. CA: All entrances/exits must be self closing and tight fitting to prevent the entrance and harborage of insects.

Employee stated that neither has worked since they began working there. CA: The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment, such as litter or equipment that is nonfunctional or no longer used. If equipment is to be used in facility it must be maintained in good repair and working.
