Observed establishment did not have a copy of the CFSM certificate on file. Upon further conversation with the PIC he stated the owner has the certification. Inspector attempted verifying the certification and was unable to find a valid certificate. CA: CFSM certificate shall be posted in public view in each food service establishment.
18 - insects, rodents, and animals not present

Observed back door was not tight fitting. CA: Outer openings of a food service establishment shall be protected against the entry of insects and rodents by having preventive measures such as tight fitting doors.
16A - hot and cold water available; adequate pressure
Regulation: 511-6-1.06(1)(g),(h) - water supply, capacity; pressure (pf)
Observed front hand wash sink would not provide hot water. Informed PIC that the facility has been warned multiple times about the same sink and the hot water being shut off. CA: Water under pressure shall be provided to all fixtures, equipment.
