Routine
4-1A - food separated and protected
Regulation: 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c)
Observed metal tub containing raw chicken stored above raw shrimp in walk in cooler. Food shall be protected from cross contamination by arranging each type of food in equipment so that cross contamination of one type with another is prevented. COS: PIC moved raw chicken so that it was not above the shrimp.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Upon entering the facility. Observed multiple wet wiping cloths stored on counter tops. Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). COS: PIC placed the wiping cloths in a sanitizer solution.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed large amounts of excess items no longer being used stored in the kitchen area sufficient to create harborage conditions for pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by eliminating harborage conditions. PIC needs to remove any and all unnecessary clutter or equipment that is no longer in use stored in the facility.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed buildup of grease and debris along the bottom of the wall and floor throughout the facility. Observed buildup of grease along the bottoms of the grill and fryer equipment. Observed excessive amount of grease buildup on the sides of equipment next to fryers. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC needed to thoroughly deep clean the entire kitchen area.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed buildup of debris on floors and walls throughout facility. Observed unnecessary clutter stored throughout kitchen area. Observed missing sheet rock on wall in men's bathroom below the sink. Observed missing ceiling tiles in kitchen area. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. PIC needs to replace ceiling tiles and missing sheet rock in bathroom. PIC also needs to thoroughly clean facility.
