Freezer door for the walk in freezer needs to be replaced. CTM: Send confirmation of door being replaced to the health department.

Observed the fan guards in the walk in cooler dirty. CTM: Send confirmation to the health department when cleaned.

Observed opened bags of chili and cheese sauce in the dispenser machine being held without the machine on and below 135F. CA: All TCS foods shall be hot held at 135F or above or cold held at 41F or below. COS: The bags of chili and cheese were discarded.

Observed vacuumed sealed salmon that was already thawed being stored in the walk in cooler without opening the package or taking the fish out of the reduced oxygen environment. COS: The PIC discarded the fish.

Observed several items including boxes of cooking oil, pans on top of the boxes, beer and other items being stored on the floor of the kitchen. COS: PIC had the items moved off the floor.

Observed the sink in the employee bathroom broken off making the kitchen stink. CTM: The sink shall be repaired within 72 hrs and send confirmation to the health department.

Observed a lot of water or juice from spilled foods on the floor in the walk in cooler. CTM: Send confirmation of this being cleaned to the health department within 72 hrs.

Floors and walls have a build up of grim and need a deep clean.

The kitchen needs to be cleaned up and free of any unnecessary items that are not needed for operations in this facility.

Observed several employee items like drinks, phones, and other personal items being stored above and on prep tables along with food for the facility in the kitchen where they can potentially contaminate facility foods. CTM: Designate an employee area for personal items with in 72 hrs and send confirmation to the health department.
