Room 210- Observed stains on lamp shade in room.; Room 105- Observed stains on lamp shade in room.; Room 108- Observed headboard upholstery with large arounds of scratches. Observed tears on upholstered chair in room. Stains observed on sofa bed in room. Handles missing off dresser in room. Counter top of sink broken with large crack

Room 108- Observed stains on sheets and mattress.; Room 105- Observed stains on on sheets on both beds.; Stains on pillow.; Room 210- Observed box spring cover heavily soiled with stains. Observed stains on sheets. Observed cigarette burns in the comforters and sheets

Room 108- Observed evidence of water damage on room in ceiling.; Room 203- Observed gap in tile around shower knob in bathroom. Room 210: Observed baseboards in need of cleaning in room 105, 108, 203.; Room 105- Observed a lot of trash under sinks in cabinet under sink. Trash observed inside tv dresser drawers. Room 210- Observed trash under bed

Room 102 &110-Decor pictures observed with moldy/mildew stains. Observed staining in upholstered sofa. Room 102- observed damage to upholstered head board, stains on lamp shades, and stains and crumbs inside sofa bed. Room 209- Observed torn upholstered chair. Room 110- Observed damage lamp shade. Observed missing knobs on night stand

Room 110- Observed cigarette burns in mattress pad on bed. Room 209- Observed cigarette burns in comforter of bed and discoloration on sheets. Also observed stains on pillows, mattress pads, and sheets

Observed hole in pillar closest to Burger King at the end of the building. -Observed missing soffit vinyl siding on second floor of motel. -Observed damage to top side of building closest to laundry room

Room 102- Observed evidence of water damage on room in ceiling. Damage/mold in ceiling in shower. Room 110- Observed evidence of water damage on ceiling. Observed hole in ceiling in shower. Room 204- Observed damage to baseboards behind door in room

Room 209- lighting and electrical not working in restroom & sink area. Ventilation vent in the rest room was observed hanging. Room 110- Observed light fixture without covering. Room 204- Observed missing smoke detector

Room 104- Staining was observed on lamp shades, upholstered green head board in disrepair with tears, small stains observed on curtains. Room 116- Upholstered chairs observed with stains, and staining on lamp shades. Room 124- Staining was observed on upholstered seat, upholstered green bed headboard in disrepair with tears

Room 104- observed stains on sheets and burns on comforter. Room 116- Observed comforter with cigarette burn holes and paint stains on the sheets. Room 124- Observed cigarette burn marks on the pillows, sheets and mattress pad

Room 124- Observed missing smoke detector with plastic cup stored in place of smoke detector. Room 116- has mini fridge that is rusted on the top of it

Room 108- Head board in need of reupholstering, chair at desk has tears in upholstery. Lamp shade in need of replacing. Room 110- observed wax stains on upholstered chair in room. Light shade in need of replacing

Room 108- Observed stains on fabric on upholstered sofa. Observed stains on mattress protectors and sheets

Observed damage (holes in stucco) to outside pillars if building on first floor closest to burger king

Room 108- Door leaded to bathroom is swelling at bottom; Observed mold on ceiling near sofa chair

Observed stains on clean and folded sheets stored in the housekeeping/laundry storage area

Facility unable to produce most current gas fired inspection report. PIC was able to product report for recharging of fire extinguishers etc. PIC stated fire extinguishers and gas fired equipment was conducted on same day. *Send documentation to Kristen.Tyler@dph.ga.gov of gas fired equipment inspection. If not conducted have gas fired equipment inspection conducted

Room 203- Was closed for maintenance. Has apparent water damage from leak in ceiling. Room 212- carpet has mildewy smell and in need of replacement. Dresser in need of knob replacement and broken leg on dresser under television

Room 212- has small amount of staining on comforters and sheets; Room 222R- has large staining on mattress pad and mattress. Multiple nails drilled outwards at bottom of bad frame which could pose safety hazard

Room 212- Dresser drawer has a lot of trash inside, needs cleaning. Clean corners of all observed rooms and in tubs

Room 214- Ventilation vent in bathroom is not operating; Cover is missing off of ventilation vent in rest room. Room 222R- mold was observed on popcorn ceiling in room in front of sink and in bathroom above shower; Room 218- was closed for maintenance. A large amount of mold was present on wall behind entry door and on ceiling. Room 203- Was closed due to roof leak and caving of ceiling. Mold was present on insulation near room leak and in freezer/fridge combo; Room 212-ventilation system is bathroom is not working

Room 214- Holes were observed in walls throughout room and paint was peeling off ceiling in corner of room near door; Room 203- was closed due to maintenance hole in ceiling due to roof leak; Room 222R- Hole in wall observed near outlet near front window; Cable outlet face plate removed and outlet was filled with cloth. Wall was dimpling near bathroom. Smoke and sut observed on top of bathroom door frame

Facility is not able to produce documentation for most current gas fired inspection report. PIC stated owner has it at personal office. Discussed with owner. *Send documentation to Kristen.Tyler@dph.ga.gov of gas fired equipment inspection. Must be kept on site at facility for viewing by Health Authority

Room 104- Comforter on bed has holes, sleeper sofa has small stains on cushions; Room 106- Curtains have staining, Clean sleeper sofa, Sheets on bed have stains, Mattress pad has cigarette burns; Room 105- Staining on mattress, Staining on box spring, Staining on mattress pad, Replace pillow cases, has spider webs.; Room 125R staining on curtains, sheets have stains, clean recliner chair,

Room 104- Shower needs to be cleaned in corners, build up of debris, room also has dirt debris in corners; Room 125R- Needs cleaning of dirt, dust, in corners of room, underneath AC unit, recliner chair needs cleaning; Room 105- Clean air vents on AC Unit; Room 106- Clean freezer portion of mini fridge excessive ice build up. Clean sleeper sofa from dirt and debris

Room 106- Ventilation vent is bathroom is non operation; Room 106- extra mini fridge in room has mold/mildew inside; Room 125R- Vent cover is missing and on tire rack;

Room 102- Observed spiders in room and well as bugs on bed; Room 112H observed spiders in room; Room 108- Observed dead roach on floor; Room 118-Observed spider webs and insects

Room 112H- Sleeper sofa had excessive amounts of dog hair; Room 108- Old gummy/candy present inside sleeper sofa; Room 102- had old hair in sleeper sofa

Room 118- Mattress was heavily soiled with large stains, comforter in room had cigarette burns and stains on sheets; Room 114- Mattress had mold and stains

Observed walk ways through out grounds soiled with litter and debris

Room 108- Fecal Matter observed in toliet, restroom needs cleaning; Room 112H bottom of toliet near floor needs cleaning

Room 112H- Light cover was off of light in restroom

Room 102- Ventilation Vent is non operational; Room 102- Mold is present on ceiling above shower; Room 114- Shower curtain has mildewy smell. Room 114- Mold is present on back of bathroom door; Room 118- Shower head is moldy/mildewy

Room 102- Hole is present in bathroom door, walls need repainting and cleaning. Room 118- Walls need cleaning and painting, missing floor board where floor in front of sink meets bathroom floor; Room 108- Ceiling in disrepair and has hole above shower, Bathroom door is swelling/splitting at bottom. Floor boards missing where flooring in front of sink and bathroom floors meet.;
