Observed cobweb in the ceiling in rooms 131, 137, 140, 211, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. The manager will have the rooms cleaned in the ceiling by 06/202/2026

Observed a missing anti-slip mat on room 137. 511-6-2-.08(10) - Anti-Slip Surface Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair.
