Observed a employee wearing a bracelet on his wrist in the kitchen while cooking, Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS the employee will remove the bracelet from his wrist when prepping or cooking in the kitchen. Correct by 05/13/2026

Observed heavy buildup of carbon on the side of the stove, Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. The manager will clean the side of the stove by 9/5/25

Observed dirt and grease buildup under the dishwasher and 3 compartment sink in the kitchen area, The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food product. The manager will have the area under dishwasher cleaned by 9/10/25

Observed a bag off noodles not properly protected against possible contamination, food shall be protected from contamination by storing the food:

(i) In a clean, dry location;

(ii) Where it is not exposed to splash, dust, or other contamination; . The manager will place the noodle in a close container between usage to prwvent possible contamination.

Observed a scoop handle buried in the flour in the plastic container, .In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon. COS the manager will have the scoop removed from the flour and stored corretly with the handle up after wash, rinse and sanitize has be done.

Observed a employee drinking from a disposable water bottle with no lid or straw, However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. the manager will makw sure the employee drinks from a single service with lid and straw. COrrect by 12/13/2024

I observed the ice scope was not covered to prevent contamination. Equipment, Utensils, Linens, and Single-Service and Single-Use Articles.

1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored:

(i) In a clean, dry location;

(ii) Where they are not exposed to splash, dust, or other contamination; and

(iii) At least 6 inches (15 centimeters) above the floor. 2. Clean equipment and utensils shall be stored as specified under paragraph 1 of this subsection and shall be stored: correct by 12/20/2024

Observed a employee drinking from a cup with no lid or straw, However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. the manager will makw sure the employee drinks from a single service with lid and straw.

Observed the employee drinking water from a plastic bottle in the kitchen area, However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. COS the manager will discard the bottles of water.

Observed bags of sugar not properly sealed to possible prevent contamination, all food need to be storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination; The manager will the bag placed in a container or wrapped completely to prevent contamination.

Observed a leaking fuacet in the kitchen area, System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair. Will fix the faucet by 9/30/23
