Observed missing NO DIVING wording in 4 inch lettering at each depth marker around the pool deck. a) The words "No Diving" and the universal international symbol for “NO DIVING” shall be permanently visible at the edge of the deck for water five feet in depth or less, placed only on the deck beside the depth markers, and shall conform to that outlined for depth markers in subsections (3)(a) thru (h) above. EHS spoke with the PIC about this as the facility has been informed several times of the need to place the wording around the deck at each depth marker 5FT and less in 4 inch lettering. She stated that the owner will not install the proper tiles as the pool was permitted with the current tiles; owner wants the requirements in writing.
Observed low calcium hardness levels. EHS informed the PIC of the need for the CPO to balance the water chemistry.

Regulation: 511-3-5-.17(1) - (1) disinfectant levels (a) free chlorine, ppm in pools not using cyanuric acid or a stabilized chlorine compound use (b),1.-5.
Observed pool with a high combined chlorine level. Facility is to balance the water chemistry as the combined chlorine should not exceed .4 ppm. High combined chlorine results in chloramine formation and reduced chemical efficacy. Take remedial action to establish break point chlorination. When combined chlorine is over 0.4 ppm, superchlorinate by adding ten times the combined chlorine in ppm and subtract the current disinfectant level.

Observed little to no chlorine present in the pool; EHS tested chlorine levels using two different test kits to ensure accuracy of the results. Per pool test logs, the CPO last checked the pool on 6/23/25 and the chlorine level was stated to be 1.0; CPO shocked the poolfor the corrective action. (a) During operation, disinfectant levels are less than the minimum level specified in DPH Rule 511-3-5-.17. If the level of the disinfectant used is not specified in DPH Rule 511-3-5-.17, the disinfectant must be approved and kept at levels determined necessary by the health authority.

Observed pool not keeping daily records for pool maintenance. PIC informed EHS that CPO is out once a week. Maintenance worker informed EHS that he doesn't know much about the pool. Pool is not being maintained everyday

Observed a gas container and a potentially gas powered pressure washer stored in the the pool pump/chemical room. EHS informed PIC of the need to remove the items.

EHS informed PIC of the need to adjust chemical reading and to have the pool serviced at least twice a day between servicing from the pool company.
