Observed several staff members handling food and performing various other tasks, including beginning work shifts without washing their hands as required. Staff must be trained on proper handwashing practices and the Person-in-charge (PIC) must ensure that staff are following these practices.
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed several food contact surfaces of equipment (pizza cutters, tongs, scoops, counters, pizza rack) with excess buildup of food product. These items are not not being cleaned/sanitized every 4 hours as required (discussed with PIC). Utensils were cleaned and sanitized on request during inspection. Discussed cleaning/sanitizing methods for other food contact surfaces that could not be addressed immediately with PIC.

Observed several flies in facility. PIC mentioned having an issue with the drive-through window (was working during inspection). Management has called a pest control company. Ensure that fly problem is addressed.

Observed several shelving units and exterior equipment surfaces, handwash stations(paper towel and soap dispensers) with food grease/debris buildup. These surfaces are not being cleaned. Establish and maintain a regular cleaning schedule and ensure these tasks are getting completed.

Employee restroom door than opens into kitchen area was open and does not have a door closer. Restroom doors must be self-closing and a closer must be added.

Observed heavy buildup of grime throughout kitchen areas on floors/walls. Facility is in need of thorough cleaning. Regular cleaning schedule must be established and maintained.

Observed sanitizer in 3 compartment sink not dispensing, therefore food contact containers/utensils are not being sanitized. Corrected on site: Bleach purchased to make sanitizing solution until Quaternary dispenser is repaired. (4 point violation)

Observed back door with no air curtain that is not self-closing. If no air curtain is to be on door, there must be a way for the door to automatically close. Repeat Violation. (3 point violation + 1 point for repeat violation).

Observed Quaternary Sanitizer not dispensing in 3 compartment sink. Maintainance company contacted and bleach purchased to make sanitizer until Quat sanitizer is fixed. (1 point violation).
