Observed dead pest and insects in several of the rooms inspected today. Observed dead pest in rooms 122, 326, 327, 305, and 406. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Must correct within 24 hours
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed several bedding and linens in most of the rooms inspected with stains, debris, or hair on it (110, 122, 124, 126, 326, 305, 404, 327, 406, and 427). Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Must correct within 30 days

Observed tourist accommodation permit not posted within establishment. Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. Must correct within 30 days
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

Observed caulking around the sink in the bathroom in Room 109 not in good repair. Observed residue in the shower in Room 122 and hole in the bathroom door. Observed brown stains on the floor in the bathroom behind the toilet in Room 125, and observed shower curtain with black residue. Observed pink residue on the caulking in the shower in Room 126. Observed rust on the shower rod in Room 305. Observed pink residue at the bottom of the tub and toilet in Room 404. Also observed shower surround on the outside not in good repair in Room 404. Observed moulding in bathroom in Room 406 in need of replacing. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. Must correct within 30 days

Observed ventilation system in the bathrooms in rooms 126, 326, 327, and 305 not working. Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. Must correct within 30 days

Also observed kitchen counter not in good repair in room 427. Observed several spray bottles containing cleaning solution not marked or labeled on the housekeeping cart. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Must correct within 30 days
