Observed dead pest and insects in several of the rooms inspected today. Observed dead pest in rooms 122, 326, 327, 305, and 406. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Must correct within 24 hours

Observed several bedding and linens in most of the rooms inspected with stains, debris, or hair on it (110, 122, 124, 126, 326, 305, 404, 327, 406, and 427). Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Must correct within 30 days

Observed tourist accommodation permit not posted within establishment. Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. Must correct within 30 days

Observed caulking around the sink in the bathroom in Room 109 not in good repair. Observed residue in the shower in Room 122 and hole in the bathroom door. Observed brown stains on the floor in the bathroom behind the toilet in Room 125, and observed shower curtain with black residue. Observed pink residue on the caulking in the shower in Room 126. Observed rust on the shower rod in Room 305. Observed pink residue at the bottom of the tub and toilet in Room 404. Also observed shower surround on the outside not in good repair in Room 404. Observed moulding in bathroom in Room 406 in need of replacing. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. Must correct within 30 days

Observed ventilation system in the bathrooms in rooms 126, 326, 327, and 305 not working. Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. Must correct within 30 days

Observed paint on the kitchen counter tops in room 122. Observed stains on the carpet in room 406. Observed carpet in room 427 not in good repair. Also observed kitchen counter not in good repair in room 427. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Must correct within 30 days

Observed several spray bottles containing cleaning solution not marked or labeled on the housekeeping cart. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Must correct within 30 days

Observed a live spider in corner of bathroom door entrance. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program. POC: PIC will have pest control spray room and send a receipt upon completion

Observed facility lacking gas-fired equipment inspection report. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. CA: Facility shall have 24 hours after the day of inspection to complete annual gas-fired inspection. PIC stated that the district manager has not been responsive about approving inspection. Says she will contact her again to complete inspection as soon as possible

Observed ventilation not working properly in rooms 331, 320, 306, 205, and 209. Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems. POC: Facility shall have 10 days from the date of inspection to get ventilation in proper working condition for each room

Observed ceiling stains in room 225 and mold-like stains in the bathroom ceiling of room 229. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored. COS: Housekeeper cleaned stains from ceilings

Observed ceiling lights not working in rooms 315, 306, 205, and 229. Also observed stove lights not working in rooms 326, 315, 205, and 103. Also observed bathroom light not working in room 326. All rooms shall be well lighted. When natural light fails to provide sufficient illumination, evenly distributed artificial light shall be provided to maintain a lighting intensity of not less than ten foot candles at 30" above the floor. POC: Facility shall have 10 days from the date of inspection to fixing lighting in all rooms

Observed several spray bottles on housekeeping cart without labels. Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label. COS: Housekeepers placed labels on all spray bottles

Observed rooms 209 and 230 with stains/holes in bedding. Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. CA: Facility shall have 30 days to replace bedding

Observed rooms 331, 325, 308, 305, 227, 230, 209, 202, 105 without bathroom ventilation working properly. Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. Observed several rooms (320, 328, 226, 210) without ventilation in proper working order. Toilet, lavatory, and bathing facilities shall be mechanically ventilated. CA: Facility shall have 30 calendar days to have mechanical ventilation in good repair

Observed several rooms (320, 309, 111) with unclean walls. Floors, walls, ceilings, windows, doors, and all other appurtenances shall be in good repair and shall be kept clean. CA: PIC notified maintenance to adequately clean walls. New violation

Observed several dead roaches in rooms 329, 326, 327, and 229. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Person in charge stated Orkin Pest Control will be treating facility today. Person in charge stated maintenance sprayed rooms yesterday

Observed facility lacking gas-fired equipment inspection report. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency. Person in charge made an appointment to have gas-fired equipment inspected on 06/21/2023. Person in charge stated they would send a copy of the inspection report to the Health Authority once completed
