Observed dead pest in drawer of the nightstand/dresser in Room 315. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Must correct within 24 hours

Observed stains, debris, or holes in bedding in Rooms 220, 325, 305, 402, and 410. Also observed no mattress protector/cover in Room 315. Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Must correct within 30 days

Observed bath rubs in rooms 410, 402, 207, 112, 129, and 131 with brown stains or peeling paint. Also, observed debris in and around the sink in Room 112. Observed caulking around the sink in Room 325 with black residue and in need of replacing. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Must correct within 30 days

Observed ventilation in the bathroom in room 325 not working properly. Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. Must correct within 30 days

Observed the ceiling tiles (some not all) on each floor in the hallways in need of replacing. Observed wall under the window not in good repair in Room 129. Also observed brown stain on ceiling in Room 207. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Must correct within 30 days

Observed live roaches on the door of room 122 and a live spider on the bed in room 122. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program. POC: PIC will have pest control come and spray room 122 and will send a receipt upon completion

Observed facility with outdated gas-fired inspection. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. CA: Facility shall have 24 hours after the day of inspection to complete annual gas fired inspection. PIC stated that their district manager has not been responsive in regards to approving an inspection but will contact them again as soon as possible

Observed rooms 413 and 406 without pillows. Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. COS: Housekeeper placed pillows in rooms

Observed rooms 122, 223, 204, and 306 without bathroom ventilation working properly. Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems. CA: Facility shall have 10 days from the date of inspection to fix ventilation in each room

Observed ceiling lights not working in rooms 129, 331, and 406. Also observed bathroom light not working in room 413. All rooms shall be well lighted. When natural light fails to provide sufficient illumination, evenly distributed artificial light shall be provided to maintain a lighting intensity of not less than ten foot candles at 30" above the floor. CA: Facility shall have 10 days from the date of inspection to have lights in all rooms in proper working condition

Observed microwaves in rooms 126 and 306 missing a plate. Also observed microwave in room 222 in need of cleaning. Also observed rusted microwaves in rooms 306 and 410. Also observed white stains on the chair next to the bed in room 329. All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. CA: Facility shall have 30 days from date of inspection to correct violations

Observed rooms 309, 231, 129, and 107 lacking window screens. Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Screen doors shall be self-closing and screens for windows, doors, skylights, transoms and other openings to the outside shall be tight-fitting and free of breaks. Screening materials shall not be less than sixteen mesh to the inch. Screen doors for sliding patio doors will not be required to be self closing except in food service, preparation and utensil washing areas of permitted food service establishment kitchens and bed and breakfast inns. CA: Facility shall have 30 calendar days to have window screens installed

Observed ceiling in bathroom of room 107 peeling. Also observed ceiling tiles throughout facility damaged and/or stained. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored. CA: Facility shall have 30 calendar days to fix ceiling in bathroom and throughout facility

Observed facility lacking gas-fired equipment inspection report. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. CA: Facility shall have 24 hours after the day of inspection to complete annual gas fired inspection. PIC set an appointment to have gas fired equipment inspection on 12/15/2023

Observed bedding in room 422 to be unclean. Mattress and bed coverings shall be clean and free of vermin. COS: PIC had employee change the bed coverings. New violation

Observed tub faucet not in good repair in room 226. Toilet rooms and fixtures shall be kept clean and in good repair. COS: PIC contacted maintenance to complete repair. New violation

Observed mechanical ventilation not working properly in room 226. Toilet, lavatory and bathing facilities shall be mechanically ventilated. CA: Facility shall have 30 calendar days from day of inspection to have mechanical ventilation in good repair. New violation

Observed facility lacking gas-fired equipment inspection report. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency. Person in charge set an appointment to have gas-fired equipment inspected on 06/21/2023. Person in charge will send copies of inspection report to the Health Authority

Observed rust accumulation on microwave in rooms 131, 122, 209, and 306. Observed rust accumulation on refrigerator in rooms 122, 207, 209, and 306. The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot.
