Observed dead and live bugs in room 225 in the bathroom in the shower and on the floor near the toilet. Also observed spider on bedding in room 215. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Must correct within 24 hours
8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
Observed gas-fired equipment inspection past due. Observed the last gas-fired equipment inspection on file at the tourist accommodation conducted on 07/31/ 2024. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency. Must correct within 24 hours
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation

Observed dresser in room 230 not in good repair. Observed dressing missing the dresser door. Observed the microwave in room 225 in need of replacing or not in good repair. All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. Must correct within 30 days
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed stains on the bedding and pillowcases in all of the rooms inspected today. Observed bedding either with stains or holes in it. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Must correct within 30 days
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

Observed brown substance/rust in the toilet in room 215. Observed pink soap residue on the floor of the shower in the bathroom in room 215. Observed brown stain on the floor in the bathroom near the toilet in room 230. Observed the shower curtain in room 225 with brown and pink stains on it at the bottom. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. Must correct within 30 days
19 - anti-slip tubs/showers, slip strips, mats or appliqué products provided and in good condition

Observed anti-slip mats, slip strips, or other applique missing in the showers in rooms 215, 230, and 229. Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair. Must correct within 30 days
