12A: .04(4)(r)Observed a bag on onions, a container of sauce and a container of rice being stored directly on the floor. Food shall be protected from contamination by storing the food:
- in a clean dry location; (C)
- where it is not exposed to splash, dust, or other contamination; (C) and
- at least 6 inches (15 cm) above the floor. (C) Corrective Actions: Person in Charge (PIC) moved food items so that they were not stored directly on the floor.

17D: .07(4)(b)Observed employee food items being stored in the reach in cooler in the kitchen with items for the facility. Also observed employee personal items such as a phone and earbuds, being stored on dry storage shelf with items for the facility. Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. (C) Corrective Actions: PIC moved employee items to a designated areas.
