Note: All cold holding and hot holding temperatures were adequate. Note: Must have a notice to customers on what major allergens are served in the facility. Note: Must remove sticker that covers the top of new stainless steel table so that table can be adequately cleaned. An unscored informal will be conducted on or before 5/12/2026. If all violations are not corrected, then the permit maybe suspended.

12A: .04(4)(t)There is no splashguard between the hand washing sink and the vegetable prepatation sink. This was noted on the opening inspection. During preparation, unpackaged food shall be protected from environmental sources of contamination. (C)

15B: .05(3)(b),(c)The three-compartment sink utensil washing sink does not have any drainboards. This was noted on the opening inspection. Drainboards, utensil racks, or tables large enough to separately accommodate all soiled and cleaned items that may accumulate during hours of operation shall be provided for necessary utensil holding before cleaning and after sanitizing. (C)

This was noted on the opening inspection Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. (C) Corrective Actions: Must replace all broken floor tiles.

17C: .07(2)(a)The dry storage area has acoustic ceiling tiles instead of the required smooth vinyl ceiling tiles. This was noted on the opening inspection. Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. (C) Corrective Actions: Must replace acoustic ceiling tiles with light colored smooth vinyl covered ceiling tiles.

17C: .07(2)(c)The dry storage area does not have the coved base molding where the walls meet the floors. Some of front service area does not have the coved base molding where the walls meet the floors. This was noted on the opening inspection. The floors in food service establishments in which water flush cleaning methods are used shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. (C) Corrective Actions: The dry storage area and the front service area must have all areas where the walls meet the floors have coved base molding.
