Observed damaged vacuum breaker on mop sink faucet with hose attached. Repair or replace vacuum breaker by 9/23/24. Disconnect hose until vacuum breaker has been repaired.

Upon entry of rear kitchen, observed handwashing sink next to ice machine blocked with sheet pan. CA: All handwashing sinks must be maintained so it is accessible at all times for use and should only be used for handwashing. COS: Sheetpan cleared.
