2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(1)(c)17 - demonstration (pf)
CFSM does not have any awareness of the employee health policy or can demonstrate that her employees are trained in a verfiable manner on the symptoms and illnesses that conditional employees must be excluded from work if diagnosed or presenting illness with. CA: PIC must be able to show food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Employees need to be retrained within 72 hours on their responsbilities to report. DPH Employee Reporting Agreement copies supplied to PIC during inspection.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
No written procedure for vomiting and diarrheal cleanup available in the facility, no clean up kit, bleach is the only thing available to use on site for cleanup. CA: A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. Acquire a written procedure within 72 hours. Disinfectant is available against Norovirus, full clean up kit needs to be acquired ASAP.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Cups and plates in the back storage area were observed stored on the floor during inspection. CA:Except as specified in paragraph 4. of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii)Where they are not exposed to splash, dust, or other contamination; and (iii)At least 6 inches (15 centimeters) above the floor. COS: Items were placed properly on a shelf.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Multiple areas of the kitchen and bathroom floors have holes, missing tiles, and other damage. CA: Floors, walls, and ceilings must be maintained in good repair. Grant has been acquired from city to fix floors. Fix ASAP.
