Daniel Hubbard is serving as CFSM for both YMCA camps. There are two permits on record, one for each kitchen. Per Georgia food rules there can only be one CFSM per food permit. Designate a second person to serve as CFSM for the girls camp within 30 days and submit certificate to: brad.krzyzanowski@dph.ga.gov

Purchase handwashing soap for the designated handwashing sink.

No sign to designate handwashing sink. Add signage to this sink ASAP.

Warewashing machine has a significant leak, causing a slip hazard. Machine is also not reaching temperature. Use 3 compartment sink to properly sanitize dishes until this machine can be repaired.
17C - physical facilities installed, maintained, and clean
Repair door to walk in cooler to close automatically. Slip hazards from leaking faucets, devices still present. Repair immediately.
