Observed not all employee's have a signed Employee Health Agreement. C/A: Have all employees sign health agreements. COS: Employee's signed health agreements.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed mold like substance inside ice maker. C/A: Clean inside ice maker.
6-2 - proper date marking and disposition
Observed containers of gravy and waffle batter without prep and discard dates. C/A: TCS food items need to have prep and discard dates. COS: Employees labeled all TCS food items with prep and discard dates.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee drink and phones in same area as food items. C/A: Employee items need to be stored in designated areas. COS: All items were moved to a designated area.
