Routine
Cold holding TCS foods assessed. Food in freezers is frozen solid. Follow-up inspection is required. The food service permit has been suspended for due to 10 critical violations. Permit reinstated after on-site training. Must fix back door; door is not tight fitting. Paint all exposed wood and fix wall behind chest freezer. www.gnrhealth.com 770-278-7340

2-2E: .03(6)The concentration of bleach was not adequate. The bottle label showed bleach was 4.5%. The person in charge shall ensure that sanitizer used for vomit/diarrheal events shall be adequate.

4-1A: .04(4)(c)1(i)(ii)(iii)(v) (vi)(vii)(viii)Observed unwashed produce store with and above ready to eat food. Observed package frozen raw chicken stored directly on top of ready to eat foods. Food shall be store properly to prevent cross contamination. Corrective Actions: PIC relocated food.

4-2B: .05(7)(a)1Observed ice machine with mold like substance build up. Food contact surface shall be clean to sight and touch.

6-1A: .04(6)(f)Observed cold held TCS foods with an internal temperature greater than 41F in the prep top cooler. Cooler ambient air temperature was 39.0 F. Food was out longer than 4 hours. Cold held TCS foods shall be kept at 41F or below. Corrective Actions: Food was discarded.

8-2B: .07(6)(c)Observed two chemical lighters stored above food. Chemicals shall be stored in approved locations. Corrective Actions: The person in charge relocated 1 lighter and discarded the 2nd one.

10D: .04(4)(d)Observed food bottles that did not have a label. Working containers of food shall be labeled with a common name. Corrective Actions: The person in charge labeled bottles during the inspection.

12B: .03(5)(i)Observed employees preparing food without a hair restraint. Employees shall working with food shall restrain all hair that is 1/2 inch or longer.

15A: .05(6)(q)1&3Observed ice build up inside the reach in freezer. Equipment shall be maintained in good repair.

15C: .05(7)(d)Observed old food debris build-up on the side of the fryer, two small reach in coolers, fry baskets. Nonfood contact surfaces shall be kept cleaned at a frequency to preclude accumulation.

16B: .06(2)(a)The hand washing sink leaks onto the floor. The sink is coming off the wall. The plumbing shall be in good condition.

Observed an employee enter the backdoor and handle equipment without first washing his hands. Observed an employee Employees shall wash their hands at the proper times. Corrective Actions: Employees washed their hands.

The person in charge shall be knowledgeable of their employee health policy. Corrective Actions: The person in charge was provided with an employee health agreement poster and educated on reporting requirements.

Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles). (C) Corrective Actions: The person in charge discarded both drinks.

2-2D: .07(3)(a)The hand washing sink in the kitchen did not have a cleanser available. A hand cleanser shall be available at the hand sinks at all times. Corrective Actions: Dish soap was provided during the inspection.

2-2D: .07(3)(b)The hand sink in the kitchen and restroom did not have a drying provision. A hand drying provision shall be available at hand sinks at all times. Corrective Actions: The person in charge supplied both sinks with paper towels.

2-2D: .03(5)(d)Observed an employee washing their hands at the 3 compartment sink. Observed a second employee washing their hands at the vegetable sink. Hands shall only be washed at hand sinks. Corrective Actions: The employee was educated on where to wash hands.
