1-2B: .03(3)(c)Observed Facility does not have a current Certified Food Safety Manager Certificate Posted. CFSM certificate expired 06/2025. Within ninety days of the expiration of the CFSM certificate, the CFSM shall enroll in an approved food safety training course, pass an approved exam and obtain a new certificate. (Pf)
**Facility must correct violation no later than 2/13/26 or permit will be suspended until documentation is provided to the Health Authority. The person taking the training course shall take an approved course for a Certified Food Safety Manager's Certificate.

2-2A: .03(2)(m)Observed the person was not able to provide a signed copy of their employee health agreement form. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: The person in charge went over the required information and signed form onsite.
