Observed mildew and slime on inside of ice machine. All food contact surfaces should be clean and sanitized prior to use and cleaning should be scheduled accordingly. PIC COS- PIC discontinued use of this ice maker until it is properly cleaned and sanitized.

Observed several employees with no hair restraint while working directly with food in kitchen. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. PIC stated he would ensure employees wore caps or hairnets.

Observed plumbing leaks in main kitchen causing water throughout floor near ice machine and around reach in cooler. A plumbing system shall be repaired according to law and maintained in good repair. PIC states repair was made but leak continues to return- he states he will call a plumber to fix leak.

Observed floors throughout kitchen covered in water and grime that resulted from plumbing leak. Staff should be trained or re-trained on proper cleaning schedule and procedures. PIC states floors and water would be cleaned up immediately.

Observed paper towel dispenser empty and no paper towels anywhere near the handwash sink at time of inspection. Handwash sinks should be equipped with soap, hot water, paper towels, trashcan, proper signage and should be used for no other purpose. PIC replaced the paper towels upon request during inspection.

Observed soap dispenser off wall and unable to use while laying down on counter near sink in both the men's restroom and also the main handwash sink at entrance into kitchen. Handwash sinks should be equipped with soap, hot water, paper towels, trashcan, proper signage and should be used for no other purpose. PIC stated he had velcro strips to put soap dispensers back on wall today.

Observed evidence of wet-stacking. Previously mentioned during inspections- old, un-used equipment needs to be removed from drying rack in order to make room to dry equipment that is used regularly.

Observed salad cooler in disrepair. Observed dish machine in disrepair. All equipment should be maintained and in good repair or removed from facility. PIC put in work order to have cooler repaired.

Observed non-food contact surfaces of rolling carts, cutting boards (used as surfaces on top of carts), coolers (inside, outside and gaskets) throughout facility unclean and soiled with encrusted food material. Observed food debris and standing water inside interior of reach in coolers. All equipment and surfaces must be maintained clean and in good repair, in order to prevent contamination of food.

Observed air condition vents/grates soiled with accumulated dust, debris and mildew. Vents and grates must be placed on regular cleaning schedule to prevent accumulation of debris on surfaces.
