Observed floor and floor covering damaged and not easily cleanable throughout facility. Observed grease build up on kitchen and uneven and dark colored surfaces not easily cleanable.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Owner states kitchen floors will be replaced with new tile, light in color, by the end of 2025, they already have it lined up- just waiting on the workers to start.

Observed employee with beard prepping food without proper hair and beard restraint. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.

Observed 2 buckets of sanitizer reading "zero" strength chemical sanitizer. Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-4-.05(6)(m). PIC replaced water with correct sanitizer strength at time of inspection.

Observed gap at back door, holes in the walls, "attic" access point cracked. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Filling or closing holes and other gaps along floors, walls, and ceilings.

Plumbing issues were observed during this inspection, as well as during multiple previous inspections. These issues have resulted in standing water on the floor and recurring clogs in the 3-compartment sink and fruit/veg sink. A plumbing system shall be repaired according to law and maintained in good repair.

Observed encrusted food debris on equipment and surfaces throughout facility (including spilled sugar, spilled syrup packs, red substance in cooler, onion peels, dust and encrusted unknown food debris) Surfaces and equipment shall be cleaned at any time to prevent contamination of food. Facility in need of deep cleaning throughout. PIC states surfaces will be cleaned immediately, owner states walk in cooler will be removed from facility at some point.

Observed dumpster / grease receptacle area outside in need of cleaning and removal of trash/unused equipment. Observed thick grease buildup on and around grease disposal tank. Soiled receptacles and waste handling units for refuse, recyclables, and returnables shall be cleaned at a frequency necessary to prevent them from developing a buildup of soil or becoming attractants for insects and rodents. Any unused equipment should be removed from the garbage area.

Observed light in WIC flickering. Lighting should be adequate in food prep and food storage areas. Owner states entire WIC will be removed at some point. Also observed ventilation system not adequate for facility as evident with buildup of grease and smoke/fumes present. Observed a fan in kitchen covered with dust and debris. Ventilation hood systems and devices shall be sufficient in number and capacity to prevent grease or condensation from collecting.

Observed hot holding TCS (Temperature Controlled for Safety) foods below required temperature of 135 degrees or above. See temperature log for said items. All hot held hazardous food items must be maintained at proper temperatures when held hot. COS- discarded all items out of temp.
