Observed condensation from cooling unit inside walk-in cooler dripping onto food items shelved underneath cooling unit. All food items were properly covered at time of inspection, but informed CFSM that this needs to be resolved and that he may want to notify HVAC company to come out and help resolve the issue. All foods shall be protected from possible contamination.

Observed black mold in the top cracks and door area of ice machine. Recommended draining ice machine and doing a good, thorough cleaning of inside of ice machine to prevent contamination of ice due to drippage onto the ice. Other areas of ice machine appeared to be clean, but does need to be properly cleaned.

Observed bottom shelf of prep table in main kitchen badly soiled and in need of cleaning. Recommended to CFSM that items be removed from shelf so table can be properly cleaned.

Observed 1 mop stored on the floor and one inside the mop bucket in the back storage room area. Mops shall be hung to air dry.

Observed badly soiled floors in back storage room around the mop storage area and under shelves around drain. Floors, walls, and ceilings shall be constructed, designed and installed so they are easily cleanable and shall be kept free of accumulations of dust, dirt, and any other debris.

Observed a head of cabbage stored in the walk-in cooler uncovered. Food shall be stored in food-safe containers and covered and protected from possible contamination. CFSM will go over and remind employees to properly store food.

Observed soiled food contact surface of microwave oven in main kitchen. Food debris stuck to the ceiling of the microwave oven. CFSM will remind employees to clean microwave daily.

Observed that the Servsafe certification for Stephen Clark, CFSM, expired on July 26, 2023. Establishment shall have a certified food safety manager on-site within 60 days.

Observed box of lettuce stored opened and uncovered and several pieces of loose lettuce on the floor and on the shelf. Food shall be stored in food-safe containers and covered and protected from possible contamination.

Observed a gallon of whole milk opened and not marked with the proper discard date. Once opened, if not served within 24 hours after opening, TCS foods shall be date marked with a discard date of 7 days after opening, or by the use-by date from the distributor, whichever comes first.
Observed several boxes of food and food service items stored directly on the floor in the walk-in freezer and in the back storage closet. All food and food service items shall be stored at least 6 inches off of the floor.

Observed the sliding door on the outdoor dumpster left open and trash all over the ground around the dumpster area. All doors and lids on garbage receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight-fitting lids and/or doors.

Observed food debris on the floor underneath the shelving units in the walk-in cooler and freezer. Observed dirty ceiling and ceiling vent in the back storage area (directly over stainless steel pans, which are food-contact surfaces). Floors, walls, and ceilings shall be constructed, designed, and installed so they are easily cleanable and shall be kept free of accumulations of dust, dirt, and any other debris which could lead to possible food contamination.
