Observed outer openings not sealed with kitchen door having significant holes at the bottom of the door. CA: Food service establishment shall be protected against the entry of insects and rodents by doors being solid and tight-fitting door. Door shall be properly sealed and documentation of correction emailed to Carly.Harper@dph.ga.gov. PIC stated she will have someone here Wednesday to repair.

Observed handwash sinks at the server/drink area and bar not supplied with soap. COS: PIC placed soap at both sinks. CA: Keep Handwash sinks supplied with hand soap at all times.

Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit. Rice appears to have been cooled improperly yesterday. Rice was discarded by PIC. Discussed items shall be 41 F or below when cold holding.

Observed Time/Temperature Controlled for Safety (TCS) food items that are labeled as "keep refrigerated" were not discarded per manufacturer's expiration date or sell by date. Cut leafy greens were discarded by PIC.
