CA: EHS advised PIC to purchase bulletin board to have scores/permit posted for public view

EHS observed no back siphonage devices provided by facility at spigots at individual trailer spaces. EHS observed no representative at facility upon arrival. EHS called number posted on office and spoke with representative to inform them of arrival and that routine inspection conducted. EHS called a second time to obtain code for entrance to bathhouse and to get paperwork required. Representative never arrived at facility while EHS was present. Violation: Tourist accommodations shall be subject to inspection at reasonable hours no less than twice annually and as often as is deemed necessary by the Health Authority to ensure adequate compliance with the provisions of these rules. The permit holder is responsible for providing a person or persons at the time of inspection who are authorized and able to provide access to all rooms, facilities and records of the tourist accommodation, and who can demonstrate that there is sufficient daily oversight of employees and routine monitoring of operations. CA: facility shall have representative on site during reasonable hours to provide access to necessary facilities

EHS observed facility unable to provide pest control records despite attempts to contact facility representatives requesting information. Violation: A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator. CA: ensure to maintain record and make available upon request
