12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(q) - food storage (c)
Food stored on floor in WIC and WIF. Advised PIC to relocate. Food shall be stored at least 6 inches off the floor to protect from contamination.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Shelves holding equipment, ss items, bags, trays dirty. Shelf above hot hold unit dirty. Cooler doors and inside dirty. Advised PIC to clean. Food contact and non-food contact surfaces shall be kept free of grease deposits and an accumulation of dust, dirt, food residue and other debris.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.07(2)(l) - toilet rooms, enclosed (c)
Toilet room door not self-closing. Advised PIC to make self-closing. Toilet rooms shall be provided with tight fitting and self-closing doors.
17C - physical facilities installed, maintained, and clean
Floors throughout kitchen nasty under shelving and equipment with trash, liquid spills, food and grease. Advised PIC to clean. Physical facilities shall be cleaned as often as necessary to keep clean.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(5)(d) - cleaning ventilation system, nuisance & discharge prohibition, cleaned in way not to cause contamination or create a public health hazard (c)
Vents in ceiling dirty with large amount of dust. Advised PIC to clean. Intake and exhaust air vents shall be cleaned as often as necessary to keep clean and prevent contamination of food and food products.
