Observed vegetable soup dated 1-26-2026 at 50 degrees F. Foods under cold holding must be held at 41 degrees F. or below. Advised PIC to have staff cool foods in shallow pans.
Food was discarded by PIC.

Observed vegetable soup in cooler without a label on each individual container. Only the top container was labeled. Each container should be labeled. PIC added label to container.
