6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed tomatoes in Prep top cooler out of temp. Make sure cold holing foods are 41F and under. Notified PIC that tomatoes are not in temp advised to place tomatoes in RIC before slicing.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed multiple items in reach in cooler with no lids. Observed container of bread crusts and bacon with no lids. Make sure food is covered to prevent contamination. Cover items when not in use. Notified PIC to keep items covered with lids.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed build up on ice machine and can opener. Make sure to clean frequently to avoid build up on food contact surfaces. Notified PIC to clean items more frequent.
12A - contamination prevented during food preparation, storage, display

Observed chip bag on top of sliced tomatoes in PTC. Make sure the chip bag is not in contact with ready to eat foods.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed server with no hair restraint serving soup. Observed two employees with no beard restraints. Make sure all food handlers are wearing hair restraints. Beards that are longer than one half inch must wear bear restraints.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed coffee mug being used as sugar scoop with handle in contact with sugar. Scoops must be stored with handles not in contact with product.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(g)2 - kitchen and tableware, properly handled to protect food-/lip-contact surfaces (c)
Observed multiple plates, bowls, and silverware not inverted/ properly stored. Make sure to invert plates and bowls and cover silverware to avoid contamination on food contact surfaces.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used

Observed RIC door in disrepair. Make sure to fix RIC door and torn gaskets.
