Back door by WIC is not self-closing and stays open. Must keep door closed to prevent pest entry.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(i) - service sinks, numbers and capacities (c)
Observed no utility sink at facility. CFSM states they had to remove it when they were remodeling. Advised that utility sink must be placed immediately. CFSM states it should be replaced/ built by the end of the month.
13A - posted: permit/inspection/choking poster/handwashing

Observed inspection report not easily visible to consumers. Post where consumers can see inspection report.
14D - gloves used properly
Regulation: 511-6-1.04(4)(n)2,3,4 - gloves, use limitations, slash resistant gloves & cloth gloves (c)
Observed employees change tasks without washing hands and changing gloves. When washing dishes, make sure employees are handling clean/ sanitized dishes after they have washed their hands/ changed gloves, so they do not contaminate clean dishes with dirty gloves.

Room where WIC is located is not fully constructed. Ceilings, floor, and walls must be non-absorbent, smooth, and easily cleanable. Observed visible raw wood and ceiling not finished. Floor must be sealed if it is unsealed concrete. CFSM states it should be finished by end of month.
