Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels in mens restroom Repeat Violation

Basic - Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed dish machine not operating.

Basic - Employee with ineffective hair restraint while engaging in food preparation. Observed male employee with no hair guard or beard guard. Employee put on hair guard Corrective Action Taken

Basic - Hood filter missing from automatic fire suppression/exhaust system. Observed several hood filters missing above grill

Intermediate - Food manager certification expired. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/DBPR/hotels-restaurants/food-lodging/food-manager/ Observed person in charge with expired food certified manager certificate 11/8/2021.

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no towels at two hand wash sinks next to ware wash area. Also sign missing at hand wash sink behind self-serve drink dispenser. Inspector provided sign for manager to post.

Intermediate - Spray bottle containing toxic substance not labeled. Observed unlabeled spray bottle under sink at front counter.

Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed four ice buckets next to ice machine not inverted or protected.

Basic - Floor soiled/has accumulation of debris. Observed accumulation of paper, cups and other food debris in corner of kitchen and under bread trays Repeat Violation

Basic - Garbage on the ground and/or pad around dumpster. Observed cans boxes and trash bags on ground around dumpster. Repeat Violation

Basic - Missing drain plug at dumpster. Observed one of the two dumpsters missing a drain plug.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Buildup of food debris on Shake spinner Repeat Violation

Basic - Dead roach on premises. One dead roach on floor under employee break station.

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Drink on table with sugar in wait station.

Basic - Floor soiled/has accumulation of debris. Buildup of debris in corner and under equipment in kitchen area.

Basic - Garbage on the ground and/or pad around dumpster. Debris, grease buildup on and around dumpster. Also, lid to dumpster is broken.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Buildup of debris on gaskets of reach in coolers on cook line. Food debris buildup on top of dish machine. Buildup of dust/mold like substance on cardboard placed onto condenser in walk in cooler

Basic - Unclean building components, attachments or fixtures. Inside of hood vent on cook line. Buildup of grease in area behind hood filters.
