Observed : pizza sauce in use, time as a public health control sticker notes to be discontinued in use, at 2:53 am on 10-13-2025.

High Priority - Time/temperature control for safety food held using time as a public health control not discarded at the end of the 4-hour/6-hour period/or company mandated period. See stop sale. Observed : pizza sauce in use, time expired per the date/time sticker on the sauce container.

Basic - Ceiling tile missing. Observed : the internal fascia soffit tile leading into the prep area, has been displaced.

Basic - Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed 250 quaternary ammonia bucket stored with single serve wrap. The wrap was moved above. Corrected On-Site

Basic - No Heimlich maneuver/choking sign posted. Observed lack of a posted choking poster. One was found on line and printed and posted. Corrected On-Site

Intermediate - No probe thermometer provided to measure temperature of food products. Observed: there was no probe thermometer.

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed an employee drink stored over the cook line prep table. The drink was moved to the bottom shelf. Corrected On-Site

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed: the air filters for the oven components have an accumulation of dust.

High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. See stop sale. Observed marinara sauce on prep table that had been out for an hour with no time mark indicating when 10 hour time frame begin or ends.

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed mop stored in mop bucket in a manner not able to dry.

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Hand washing sink next to three compartment sink. Manager provided hand towels. Repeat Violation

Basic - Dead roaches on premises. Observed two dead roaches on floor underneath shelves where bottles of water are stored. Observed one dead roach on floor next to employee refrigerator. Manager swept up and discarded dead roaches. Corrected On-Site
