Basic - - From initial inspection : Basic - Dead roaches on premises. Observed 6 dead roaches on floor under shelf and in the corner of the hallway containing walk in cooler and three compartment sink. Observed one dead roach at entrance to walk in cooler. Observed one dead roach on floor by the mop sink. Per operator pest control sprayed the previous day. Operator swept and discarded the dead roaches. Corrective Action Taken Warning - From follow-up inspection 2026-05-07: At time of follow up inspection observed two dead roaches under flour bags and one roach in the process of dying. Operator dispose of all three roaches. Admin Complaint Corrected On-Site
