Observed 6 dead roaches on floor under shelf and in the corner of the hallway containing walk in cooler and three compartment sink. Observed one dead roach at entrance to walk in cooler. Observed one dead roach on floor by the mop sink. Per operator pest control sprayed the previous day. Operator swept and discarded the dead roaches. Corrective Action Taken Warning - From follow-up inspection 2026-05-07: At time of follow up inspection observed two dead roaches under flour bags and one roach in the process of dying. Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Cook line reach in cooler interior soiled with old food debris in cracks of base.

Basic - Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Dust pan for cleaning the floor stored on wrapped single service pizza boxes. Employee relocated. Corrected On-Site

High Priority - Establishment operating without a license from the Division of Hotels and Restaurants. Operator must obtain a public food service license from the Division of Hotels and Restaurants within 60 days. The operator has applied for license with the division but is not able to be issued a license due to ceiling tiles in kitchen area being not smooth and easily cleanable.
