Basic - Ceiling/ceiling tile shows damage or is in disrepair. Above walk in cooler ceiling tile shows water damage from a leak

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Walk in freezer

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed both hand wash sink in kitchen has evidence of old food debris in the sinks looks like something been poured in the sinks to be dumped
