Intermediate - Handwash sink used for purposes other than handwashing. Observed pitcher used to get water from handwash sink in handwash sink on cook line. Operator removed and stored appropriately. Corrected On-Site

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents above single door reach in freezer with dust build up. Repeat Violation

High Priority - Dented/rusted cans present. See stop sale. Observed 1 can of Picado Pinto Beans dented on top seam (Wt. 6 pounds 12 ounces).

High Priority - Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed triple sink at 50 ppm of Quaternary solution. Operator added 2 additional quaternary tabs to solution and remade to 200 ppm. Corrected On-Site Repeat Violation

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed paper towel dispenser empty at cook line handwash sink. Operator provided paper towels at time of inspection. Corrected On-Site

Intermediate - Proof of required state approved employee training not available for some employees. Observed employee per operator hired more than 60 days, unable to provide proof of training for employee Tolliver.
