Observed at expo line - chopped tomatoes (51F - Cold Holding); pico de gallo (52F - Cold Holding) as per operator items not portioned or prepared today, items held out of temperature for approximately 2 hours. Operator placed items in walk in freezer. Corrective Action Taken

Basic - Bathroom facility in disrepair. Observe both restroom with small amounts of water on the floor due to leak from the outside penetrating thru the wall. Maintenance employee assessing during inspection. Work order in place.
