Intermediate - Nonservice animals in the food establishment or on premises. Observed a customer who walked in to place a to go order, walk in with his German Shepherd. Customer works next door and takes dog with him. Dog has no vest or anything indicating that it's a service dog. Spoke with manager and advised to speak with customer and let him know that it is not allowed.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed hoods in pizza oven with build up of grease

Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed opening in ceiling tile around hot water tank pipe.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed on air handler dust like substance on it. Located right above 2 liter soda storage across from oven. Repeat Violation

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed multiple gaps at back door leading to the outside.

Basic - Fixed equipment exposed to spillage not sealed to adjoining equipment/walls. Observed pizza boxes underneath ac air handler which has exposed insulation hanging right above pizza boxes

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed buildup of dust on fan covers in walk-in cooler. Repeat Violation

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed wet nesting of pans on shelf above 3 compartment sink.
