High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. An employee handled their hair then put on gloves and handled a container flour tortillas without washing their hands. Educated the employee on proper hand washing. Corrective Action Taken

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. There are no paper towels at the handwashing sink on the front counter.

Basic - Cutting board has cut marks and is no longer cleanable. There is a green cutting board at the triple sink with deep cut marks and staining.

Basic - In-use ice scoop stored on soiled surface between uses. The ice scoop is stored on top of the ice machine. Operator moved the ice scoop to the triple sink. Corrective Action Taken

Basic - No conspicuously located ambient air temperature thermometer in holding unit. The digital probe thermometer is inoperable.
