Intermediate - Handwash sink not accessible for employee use at all times. Observed hand wash sink blocked by small table. Operator moved table to allow access. Corrected On-Site

Basic - Bowl or other container with no handle used to dispense food. Observed a small cup with no handle used to dispense sugar, flour and wheat germ. Operator removed and replaced with proper scoop. Corrected On-Site

Basic - Raw fruits/vegetables not washed prior to preparation. Observed banana's being cut through skin without being washed. Educated operator and operator washed and remade product. Corrected On-Site

High Priority - Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed 00 ppm chlorine off the surface of a plate after being ran three times , operator set up manual sanitation in the three compartment sink.

High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed the food service license had expired on 12/01/21.

Intermediate - Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Observed no consumer advisory. Emailed operator with consumer advisory. Observed it being printed and posted. Corrected On-Site

Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed hot water 86°F at hand wash sink in women's restroom after being ran for five minutes .

Intermediate - Whole fruit displayed for self-service not wrapped and no utensils provided. Observed apples for self service not wrapped.

Basic - Food stored on floor. Observed food stored on floor in walk in freezer. Operator moved food to shelf. Corrected On-Site

Basic - Silverware/utensils stored upright with the food-contact surface up. Observed forks stored upright behind bar area.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container in fruit for smoothies. Operator moved scoops. Corrected On-Site
