Intermediate - Handwash sink not accessible for employee use at all times. Prep line hand wash sink blocked by three boxes on floor. Employee removed. Corrected On-Site

Basic - Food stored on floor. Container of cooking oil on floor near fryer. Manager moved to a shelf. Corrected On-Site

High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Three live flies observed at front prep line and back kitchen area.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Mold-like substance on upper and side plates of ice machine bin.
